
The application forms are available in two formats: Microsoft Word and
Portable Document Format, or PDF. Due to Microsoft Word's cross-platform
capacity, the application form in Word should be viewable on recent
versions of both Windows and Macintosh operating systems.
PDF
files may be viewed in Adobe Acrobat Reader (viewing software available
for free download by clicking
here), and printed out for completion on a typewriter. We apologize
that the typewritten form is the only format currently available outside
of Microsoft Word.
Applications
have been formulated for Members, Gold Associates, and Silver Associates.
Applicants for Ivory Associateship may file an email
form submission directly to the Registrar outlining their qualifications
(the initial submission will be a summary proposal, to be expanded upon
when the Registrar emails you a reply). Applicants submitting supplemental
ancestors should use the Membership Application, writing the word Supplemental,
along with applicant's Society Member number, in the upper right-hand
corner of the first page.
Please select a form to complete. We recommend right-clicking on the link, selecting "Save Target As..." and saving the document directly to your hard disk. We have also compiled an instruction sheet (PDF format) for completing the forms; please download it and look it over before beginning.

mail, along with all necessary documentary proofs and your application
